Sometimes at swim meets, changes need to be made in the computer meet database. This form provides a written record of the requested change and required approval. When presented with a completed form, including approval the admin official or computer operator makes the change in the computer. If desired, a record of the change may be kept on the Scratch Record. Once properly recorded the Computer Change Form is filed with meet paperwork.
This is a generic Computer Change Form. The Microsoft Word format makes it easy to modify if desired. Not all meets use this form.
